This form is used to cancel your membership at Healthworks. Please ensure you are aware of the following membership conditions when cancelling.

- There is a 14 day cancellation period. Please provide as much notice as possible

- As Healthworks have no membership contracts there is no minimum number of payments, therefore, you can cancel any time by giving 14 days notice. (excludes Caloundra)

- You can still train in your club up until your membership end date. (This will be specified in your confirmation email)

- For upfront memberships, we are sorry, there will be no refunds after your 48hr cooling off period. If your circumstances change you have the option of transferring your membership to a friend or family member, or another Healthworks club. To do this there is a $50 transfer fee.

- If cancelling within 48 hr cooling off period please contact the club directly.

You will receive a confirmation email within 48 hours of submitting your cancellation. If you do not receive confirmation please contact your club as your membership may not be cancelled. If you do not have an email address please ask staff for a copy of the Cancellation note.
